Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. While applying the brake at the stop light. (25 to30 mile speed limit zone) The Toyota hybrid sienna start to accelerate for split 5 or more seconds. I have to apply the brake harder even though I was already applying the brake to slow down because the light was red and the are several cars in front of me. For the first time I was scared thinking about rear ending some one in front of me or the Toyota Lexus incident where it accelerated and everyone died flashing through my mind. The driver's side mirror vibrates when the vehicle is at speed. I've been to two dealers who say that it's normal, and everything is intact and installed correctly. My 2024 toyota sienna suddenly stopped for 2-3 seconds while driving in the city roads. I was caught off-guard with that behavior. After approximately 3- 4 seconds while depressing the accelerator pedal several times, the vehicle eventually accelerated as needed. The vehicle was driven to the residence. There were no warning lights illuminated. The vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was not informed of the failure. When a lever is pulled, the second row seat can move freely back and forth on a track until the back of the seat is returned to the upright position. I was driving at highway speeds and had to brake suddenly. The seat was at the back of the track with the lever pulled, and it slammed into the back of the driver seat, hurting my back. It stuck there putting pressure on the seat (which could be felt) until I stopped and had to use force to make it slide back.Owner report · Seats · Oct 2025

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

EQUIPMENT ADAPTIVE/MOBILITY:WHEELCHAIR RESTRAINTS/SECUREMENT:LATCH/ANCHOR:25V876000 · Dec 2025

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.
