P0721 is the ragged-signal version of P0722: the output speed sensor talks, but its story does not add up - dropouts, spikes, or values that disagree with the input shaft math. Intermittent electrical faults love this code, which is why it so often appears and disappears with weather.
A scan tool turns this from guesswork into observation: graph input shaft speed, output shaft speed and vehicle speed together on a drive. Clean input trace with a glitching output trace convicts the output sensor or its wiring; both traces glitching together points at power or ground shared upstream. “I was driving at 65 mph when my 2015 F150 shifted into 1st gear. I started skidding sideways and almost flipped but was able to run through the gears quickly and got it into Neutral.
