“[XXX] 's my wife we're driving in Sacramento.The vehicle was stopped at a traffic signal waiting for the light to change. Suddenly, without warning the vehicle started violently shuddering/shaking, scaring us completely. The shaking stopped when my wife put into park. But for those few seconds it was extremely scary. This incident should be investigated by the NHTSA as a repeat of the incident in moving traffic may cause severe property damage, serious injury and possible loss of life. “I am experiencing ongoing issues with my 2023 Lexus RX500h related to the powertrain/transmission. The vehicle has exhibited abnormal shifting behavior, including hesitation, jerking, and inconsistent acceleration. At times, the vehicle does not respond smoothly when accelerating, which raises safety concerns, especially when merging into traffic or driving at highway speeds. The issue occurs intermittently and has persisted despite multiple attempts to diagnose or address the problem. “This submission is a supplement to a previously filed complaint. Original complaint number: 11719553." In my previously submitted complaint, I incorrectly stated that the vehicle remained stationary until delivery. The actual situation is: the sunroof glass shattered while the vehicle was parked, but the day after the incident, I drove the vehicle to the dealer for inspection. I am providing this supplement to clarify the vehicle's status in the original report. “The 2023 Lexus RX-350 has had complete 12 volt battery failure twice in the last year. The hybrid battery charging procedure for the 12 volt battery is insufficient. Twice I have been stranded. The frequency and duration of the hybrid battery charging is not sufficient. A more powerful 12 volt AGM battery is needed. The dealer says I am not taking sufficiently long trips to charge the battery. Last Saturday I took two 30 - 40 minute trips but the battery had only 11.8 volts requiring a 100% charge. “On at least 7 occasions the driver was TRAPPED inside the vehicle. The car failed to start and the doors and windows locked. If the driver had not had a cell phone to make an emergency call he would have been locked inside until someone discovered the problem. Had this happened in Phoenix, AZ in the summer, there is a very good chance the driver would have been dead before he was rescued. The last time was December 11, 2025. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023 Toyota Corolla, Corolla Cross, Corolla Cross Hybrid, Highlander, Highlander Hybrid, Tacoma, Lexus RX350, 2023-2024 NX250, and NX350 vehicles that may have been incorrectly repaired under previous NHTSA recall number 23V-480. An electrical connection inside the steering column's spiral cable assembly may be insufficiently welded, causing the connection to separate and deactivate the driver's air bag. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
Remedy: Dealers will inspect and replace the spiral cable assembly as necessary, free of charge. Owner notification letters were mailed March 17, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA02. Lexus' number for this recall is 25LA02.
